We request that you bring any x-rays or MRI films, insurance card/Workman’s Compensation information, and a list of current medications with you to your appointment.  Insurance co-payment is due at the time of service.

If you are unable to keep your appointment, please notify our office within 24 hours prior to your appointment.  Appointments not kept may not be rescheduled.  For your convenience, our office requires 72 hours notice for prescription refills.  If questions remain, please call us at (315)782-9364.
